Home and Community Assistance

Often referred to as Homemaker, this service provides instrumental activities of daily living for participants in their home. It provides services to clients involving companionship, personal care, transfer assist, errands, cleaning, and other housekeeping functions.

Personal Attendant Care

Caregivers, personal care aides, or companions, commonly referred to as Personal Attendants play a vital role in assisting individuals who are elderly, mentally disabled, chronically ill, physically challenged, hospice patients, or patients undergoing recovery.

Non Medical Transportation

RFS Non Medical Transporters will provide reliable and safe transportation to and from medical, treatment, and ancillary appointments in a safe, secure and professional manner.

Case Management

RFS Case Managers will act as an advocate for you within healthcare and social settings. Their main objective is to ensure your overall well-being by providing guidance and support throughout the various processes and regulations associated with your case.

Structured Family Care Level 1-3

SFC provides financial and supportive services for family (and non-family) caregivers empowering caregivers to care for seniors and those with disabilities who wish to remain at home or in the home of an approved caregiver, rather than become institutionalized in a nursing home.

Structured Family Care Level 1-3

In-home care encompasses both medical and non-medical support provided within the home; however, it is generally associated with non-medical services such as companionship, homemaking, and personal assistance. This type of care enables your loved one to receive necessary support while maintaining their independence.
A person's eligibility for Home Care services is determined by the payer source. Typically, the patient must meet the following criteria:

  • Be homebound, even if only temporarily
  • Be under a physician's supervision with a valid physician's order for care
  • Require non-skilled assistance- Need intermittent care that is deemed reasonable and necessary for their condition
  • Be capable of safely receiving services in a home environment.
Home care services are included under Medicare, Medicaid, and most private insurance plans. RFS is here to assist you in understanding your benefits associated with paying for home care.
Reset Family Solutions facilitates the arrangement and payment for services by managing your documentation and coordinating with your payor to evaluate coverage options. We also handle direct billing to the appropriate plan and can assist you in obtaining reimbursement from long-term care insurance providers.

Medicaid is a state-run program that offers health insurance and medical support to individuals, including families and children, who qualify based on their income.

Private pay For those who prefer to pay for services independently of health insurance, private pay options are available, and Reset Family Solutions is willing to negotiate payment arrangements with you.

**In the near future, RFS will be contracted to accept Medicare, VA benefits, and Workers' Compensation**.
No.  Your personal physician will refer Home Care services and will continue to oversee your medical treatment.  The home care team will engage in continuous communication with your physician to ensure that you are receiving optimal care tailored to your individual requirements.
Caregivers play a vital role in assisting seniors and other adults in their home, ensuring their physical health and social engagement. They provide companionship, personal care, and a range of services aimed at improving quality of life. This includes support with mobility and safety, meal preparation and housekeeping, as well as transportation to activities that foster connection, purpose, and happiness.
Services can be scheduled for durations ranging from just a few hours per visit to a full 24 hours split between multiple care team members, available seven days a week, throughout the entire year.
At RFS, we advocate for the family partnership model to deliver the most thorough care for our clients.  This model presents families with two caregiver choices:

1.  We will hire your qualified family, friends, etc as your caregiver once they have completed all the eligibility requirements.  

2.  We collaborate with local organizations and educational institutions to identify qualified professionals who are eager to become Direct Services Providers (DSPs). Interested individuals can submit inquiries through our website, and upon approval, Reset Family Solutions will send them an application. We will then share the information of qualified candidates with families for interviews and potential hiring.
Home care services are offered on an as-needed basis, with visits tailored to the individual requirements and preferences of the patient. Should your physician identify a need, and this is incorporated into the care plan, visits can be arranged around the clock.
Home Care may start promptly after the patient's physician submits a formal request or referral.  An RFS representative will aim to visit the patient within 24 hours of receiving this referral, contingent upon the visit accommodating the schedule and needs of the patient, family, or primary caregiver.
RFS holds a license from the state of Indiana and is certified by Medicaid. We adhere strictly to all regulations established by both the state and Medicaid. Additionally, RFS conducts thorough background checks, reference verifications, and employment screenings to guarantee that we employ only the most qualified professionals to assist you or your loved one. 

We will also perform regular and random assessments of the care quality you receive, along with patient satisfaction surveys to be completed by you, to ensure that RFS is providing you with the highest level of service.
