Description

The primary responsibility of a Case Manager at Reset Family Solutions (RFS) is to act as an advocate for their patients within healthcare and social services settings. Their main objective is to ensure the overall well-being of their clients by providing guidance and support throughout the various processes and regulations associated with their case. Case management encompasses a wide range of specific tasks and activities that are aimed at coordinating and integrating all the necessary services outlined in the individual’s service plan.

Requirements

Requirements and Skills
  • Proven work experience in case management, including Health Care Manager or a related job
  • Excellent knowledge of case management principles, healthcare management and reimbursement.
  • Effective communication skills
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Compassionate with teamwork skills
  • BS degree in related field or social services.

Documentation Requirements
  • Must document all activities on behalf of individual being served within seven days of service.
  • Services must be outlined in service plan
  • Evidence must be provided that individual requires the level of service
  • Document services render within the RFS policy guidelines
  • CM must send out the LOC review form within 10 working days of issue, document it in the CM database system the date the form was delivered.

Responsibilities

Job function and Responsibilities 
  • Assess eligible individuals to determine eligibility for services, functional impairment level, and corresponding in-home and community-based services needed by the individual.
  • Coordinate and provide care that is safe, timely, effective, efficient, and client centered.
  • Handle case assignments, draft service plans, review case progress and determine enhancement or termination of services based on client needs.
  • Help clients achieve wellness and autonomy.
  • Periodic updates of care plans
  • Annual and quarterly face to face reassessments of individual’s needs.
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Adhere to professional standards as outlined by protocols, rules and regulations
  • Record case information, complete all necessary forms and produce reports in a timely fashion.
