Description

A Structured Family Care job involves providing care and support to individuals with disabilities or special needs in a family setting. This person is responsible for providing day-to day-day support (hereafter known as principal caregiver) may be a nonfamily member or a family member.

Job Summary:
 
  • Provide personalized care and support ot individuals with disabilities or special needs in a family home setting.
  • Create a safe, nurturing environment that promotes independence, socialization, and community integration
  • Develop and implement individualized care plans to meet the unique needs of each person
  • Assist with daily living activities such as personal care, meal preparation, and medication management.
  • Encourage and support individuals in achieving their goals and aspiration.
  • Collaborate with professionals, such as social workers and therapist, to ensure comprehensive care
  • Maintain accurate records and communicate effectively with family members and professionals.

Requirements

  • High school diploma or equivalent required; degree in a related field (e.g., social work, healthcare) preferred
  • Experience working with individuals with disabilities or special needs
  • Ability to provide care in a family setting
  • Strong communication and interpersonal skills
  • Ability to develop and implement individualized care plans
  • Basic life support and first aid certification (or willingness to obtain)
  • Reliable transportation (if applicable)
